    When I say it's failed me, I mean I get unrelated search returns or only related in the fact that I used USB as a search term and something isn't working outright.

    So, some groundwork: I'm a flight simmer. I have a joystick, throttle, rudder pedals, and head tracking (TrackIR). I also have a pair of Cougar MFDs. All but the TrackIR are plugged into a powered USB hub.

    So at some point in the past I started having issues with the rudder pedals refusing to be recognized after plugging them in. Windows would recognize that a USB device was plugged in, but nothing changed, not even in device manager. At first I thought it had something to do with device being made by CH, a company that has changed literally nothing about its hardware for 25 years and settled on the fact that a restart was needed to fix the issue. Reinstalling drivers didn't seem to help. Plugging the pedals into a different port on the case itself didn't help. Then the issue went away for a while and recently started acting up again.

    This time I was in-game when I started futzing with the CH Pedals. They were plugged in, but not recognized. When I unplugged them, I noticed a device connect/disconnect message popping up saying that the TrackIR device was disconnected. And that was no lie; the TrackIR unit was indeed shut off. Yet device manager didn't show TrackIR being disconnected at all. Furthermore, disconnecting the joystick and the throttle ALSO resulted in the TrackIR connect/disconnect message, but they were functional.

    Anyone have the faintest clue what the hell's going on?

    tl;dr plugging pedals in occasionally results in not being recognized and not appearing anywhere, unplugging them causes another USB port to react as if it was unplugged and vice versa. sos

    boot cd

    then the list (step by step)

    Next...

    USB Drivers....
    Load USB Drivers with no386.....

    Ask you load cd drivers. NO
    Ask you load SCSI drivers. NO
    Ask you anything NO.
    USB driver list comes out. 8 Don't choose anything else.

    type
    C:\ (that should be your mp3 player if you did it right)

    atiflash -i
    to list the adapters

    atiflash -p -f 0 ???.bin

    done.........

    Why would a USB cable extension fail?

    I had my Logitech 518 mouse connected to a 600mm USB extension cable, and it appears that the cable has failed, as just plugging the 518 into the USB port works as normal.
    I was constantly getting that USB connected sound you get when you plug a USB device in.

    Isn't it kind of strange for a simple USB cable to fail?...I've never had a VGA, DVI, HDMI or any RCA fail on me in the history of my computing/TV's, so I was rather surprised to discover it was the culprit.

    Formatting USB stick/usb hdd issues

    Drive has probably failed. Try this.

    Run admin command prompt and type

    diskpart

    list disk

    select disk n (n is number of usb drive)

    clean (wipes drive - do not select wrong drive!)

    create partition primary

    select partition 1

    format fs=ntfs quick

    assign

    exit

    exit

    If any step fails, drive has almost certainly failed.
