Windows 10: 4th monitor not showing up

im using a Windows 10 Pro 10.0.19045 Build 19045 on a Lenovo Thinkpad P53 with NVIDIA Quadro RTX4000 display adapter and for some reason 4th monitor is not showing up/blank. on the display properties i can see all 5 displays including laptop's display but cannot re-arrange monitor 2. when i tried to click on monitor 2 and extend desktop to this display, image will be shown in monitor 2 but i will lose the other monitor number. its just basically displaying 3 monitors and laptop display. device drivers are all updated.

4th monitor is not working

My workaround is to use PowerPoint Screen Recorder but this need additional screen capture.

The reason for Screen Recorder is Windows 10 Mouse Cursor for Office Menu item disappear when select Screen Capture App. It is cumbersome to show Tech Support what is selected on Office App on captured screen...
