BSOD during game but then goes to sleep?

Okay so Odd thing happened to me just now. I was playing a game Metal Gear Solid V to be exact and after a cut-scene it gave me a blue screen but instead of crashing and restarting it then goes to sleep. SO I press the power the button and no picture is coming on. So I then press and hold the power button to force shut it down and boot it back up after a few seconds.

However after I run the who crashed app, no crash dumps are shown in the logs. Oddly enough on Friday I had an incident where mu USB ports stopped working after I was done playing Yakuza Kiwiami, I was using an Xbox 360 wireless adapter and controller for both games, yet when I pulled it out on Friday there was no sound notification of disconnection. I plugged the adapter back in and nothing no light. I then tried a USB stick and still nothing wouldn't even show up. SO I looked at device manager and it said that there was no driver installed for my controller. So I decided to put it to sleep and turn it back on but then no screen was coming up, just a black backlit screen. So I had to hard shut it down and turn it back on.

So my question is that is it the adapter that's causing it? Or is there something else I should do?

    :)
