File structure is a mess after fresh install

I have recently installed Windows 11 and performed updates. I was having trouble with pinning Documents and Pictures folders to Quick Access, it wouldn't work. Before that, OneDrive had hijacked every folder and didn't let me have my Documents etc locally, everything was on the cloud storage, every Documents folder sent me the cloud version of that so I had to uninstall OneDrive to get rid of that. After trying several things to pin Documents and Pictures into quick access I realized this mess Powershell and vsc folders were created by me: Why do I have 5 different Pictures, 3 different Docu

    :)

    Extracting Zip Files

Are the step-by-step processes identical? That is, the folder extracted from and the folder extracted to belong to the same hierarchical file system structure. If they are, that suggests mildly the Cumulative Update left an electron or two out of place.

Fresh USB Windows 10 Install - Failing

I am trying to install a fresh copy (reformatted the hard disk) of windows 10 using a USB

    • Created an installation media on a USB flash drive (Win 10 pro) from this page https://www.microsoft.com/en-us/software-download/windows10
    • Changed the boot order on the other PC so that it boots using USB
    • Selected custom install
    • Installing Windows goes through the following steps successfully: Copying Windows Files, Getting Files ready for installation, Installing features, Installing updates and before the last step (Finishing up), it restarts
    • Then it goes back to the start of the installation steps

I am stuck and have read numerous posts but don't know what to do to install a fresh copy of Win 10.
