Windows 10: Keep getting authenticator login prompts

Hello Microsoft community, For the past few weeks I've been getting automatic authenticator prompts. I thought that someone somehow gained access to my password. Very unlikely because I use a password manager and a VERY strong password. Upon logging into my account it turns out I do not need to enter a password but simply enter the authenticator code only. OK so I googled how to turn this off. When I looked under Security >> Advanced Security Options >> Passwordless Account. - this option is not turned on and set to off See screenshotI looked at my login history and it appears

Unable to get authentication prompt for login when connected to open Wi-Fi network.

Hello, I am using Nokia Lumia 730 Dual Sim phone. It has latest software (WP 8.1Update Lumia Denim OS version 8.10.14234.375) and firmware (02040.00021.15235.53005) update installed.

The problem is that when I am connecting to an open Wi-Fi network, it gets connected but doesn't redirect to authentication login page. I have tried manually login to same page but that doesn't work. I even did hard reset my phone. On the other hand, my
friend has same phone Lumia 730 and whenever he connects to the same Wi-Fi network, he immediately gets authentication prompt. It simply opens login page in Internet Explorer after a warning that connection is unsecured and entering the username and password
gets him connected to internet. Both phones have latest and same OS. My phone used to get prompt earlier but suddenly it stopped giving authentication prompt. I don't know why. I've really tried everything. Please help me. Its really frustrating. Any help
would be very appreciable.

I have a valid Microsoft account and a Windows 10 PC with Hello/PIN account login enabled. I also have the Microsoft Authenticator app installed on an iPhone and configured for the Microsoft account. Additionally, I have two step authentication configured
for the Microsoft account. With all of that said, I have become very exasperated while trying to unravel the various authentication paths (and their security implications) through this multi level login security scheme. I find the associated account security
documentation to be woefully inadequate. Thus, I would like to pose the following list of questions for starters.

1. Does Windows Hello/PIN amount to a two step verification login procedure for my Microsoft account?

2. Does approving a Microsoft Authenticator login attempt (without two step verification explicitly set up)

amount to a two step verification login procedure for my Microsoft account?

3. Does approving a Microsoft Authenticator login attempt (with two step verification explicitly set up)

amount to a two step verification login procedure for my Microsoft account?

4. If answers to both 2 and 3 are yes, then what does explicit set up of two step authentication do?

5. Does a coherent, comprehensive document explaining Microsoft account security configuration exist somewhere?

6. If the answer to 5 is yes, wherer is it?
