Windows 10: Performance Monitor monitoring irregularly

Good day.This is for Windows Server 2012 R2 Standard.This device has started to record irregular Performance Monitoring and I am not sure how to fix this. Please see picture.Here you can see how irregular it records, sometimes the red line that appears horizontally would not even show up at all for a long period.This is not because the CPU is running at 0%.Please let me know if anyone has seen this and were able to fix it.

Performance Monitor is stuck at collecting data for 60 seconds

Kindly follow the steps below on how you can open the Performance Monitor:

• 
  Click Start , click in the Start Search box, type
  perfmon , and press ENTER.
  
• 
  In the navigation tree, click Monitoring Tools , and then double-click
  Performance Monitor .

Trouble with 2 monitors

I suggest you to change the settings to Extended Monitor and check if it helps.

Please follow below steps:

• Right Click any ware of the Desktop, and launch Personalization.
• Go to dual monitor settings under personalization, and change it Extended Monitor.

taskhostw.exe error in Performance Monitor

I've gone through all the steps to remove the potential taskhostw.exe malware and it still occurs in my Performance Monitor when I run a System Diagnosis check. It's still coming up as "Process taskhostw.exe [ProcessId: 2068] has a high CPU consumption of
99.3%".

Is there anything one may suggest to fix this?
