I may have found the solution pending the test of time.

What first led me to investigate this issue of the Rich Media Player Preview displaying in the upper left of the screen was an empty tray icon for it after playing a video and nothing else. There was no preview in upper screen. This was because I leave Explorer
open with a very small window and the Preview Pane enabled. As soon as I enlarged the size of Explorer's window, the Rich Media Preview for the selected file reappeared in the upper left outside of Explorer.

Next, I disabled the Preview Pane in Explorer and closed Explorer. Then reopened Explorer and enabled the preview pane which resulted in the Rich Preview rendering correctly for the selected video in the Preview Pane and not outside of Explorer in upper left
of screen. The task bar icon for Rich Media Player Preview no longer appears either. Now time will tell if this remains fixed.

So as to avoid the issue, use a different player, in particular for at least mp3's.

I had a different player associated with mp3s, so had not seen this. If I associate mp3's with WMP, I see this:




Interestingly, adjusting the size of the Preview pane changes the size of that player window.



while clicking on the small arrow gives this: (stereo???)




Easiest way is to use a different player for mp3s. (Note, if you do that e.g. for avi's you lose the ability to play the medium in the Preview Pane.

I was fiddling around with preview, and had previewed an AVI file. I then reset the association with mp3 back to media player, and found the preview of the AVI remained visible.

What led me to investigate this issue was seeing an empty white icon for Windows Rich Media Player Preview in the Task Bar and no preview window anywhere. The Preview Pane was empty for selected video file.

The cause was having Explorer open with Preview Pane enabled in too small of a window. Once the window was dragged open far enough, the preview window appeared in the top left of screen outside of Explorer.

Next, I disabled the Preview Pane and closed Explorer leaving the window at the same enlarged size. Reopened Explorer and selected the video file and Preview Pane displayed the Rich Media Player Preview correctly and no Task Bar icon.

It appears that in my case having too small of an Explorer window open with Preview Pane enabled causes the player preview to escape the Explorer Window container.
