Windows 10: Why did an old email get a sign in code and why did it say I signed in at my IP at a time...

Hello,I have 2 step sign in turned on and last month, I started receiving numerous emails with sign-in codes. All the attempts were unsuccessful and were from all over the place. I changed my password very frequently at first pretty much daily and then I changed the way I log into my Microsoft account so that old email that was receiving the codes should not even be associated with the account anymore.This worked for a while but last night I received another sign in code to this old email, which should not be getting Microsoft emails anymore and is no longer an alias.To make matters more

Cannot sign in microsoft account.

My device: Lumia 520. GDR2.
I cannot sign in my Microsoft account on my device. It didnt say any error but Microsoft account is not listed in Email and account list. When I click add button, it will show microsoft account option but after press sign in, it is still not shown in email
and account. Another problem is i cannot delete facebook account. Error code: 0x83cf1202. I have tried as suggested here: http://support.microsoft.com/kb/2475837/en-us#step0x83cf1202 but as my Microsoft account is not shown, I cannot do step 6.

Any suggestion?

 

Sign in issues and old Email

Why when I sign in do I still see an option to use an old email that I have removed! That email was hacked! Also, the only email showing under my profile is the one I am using but when I look at activity log in page it says it is an alias? It is my primary email and shouldn't be listed as alias. My computer has been serviced and all spyware removed. Please advise
