Windows 10: Why does my laptop erase all the data on my hard drive?

My Laptop has erased all the data on my hard drive twice now, and I don't know why nor how to fix this problem. Both times after erasing everything it tells me I need to reformat the drive again. I've tried formatting thru file explorer and thru disk manager and neither stopped it from needing reformatting. I don't understand computers that well and I don't know what i need to do to fix this and prevent it from happening again.

Hard Drive Data Recovery

got a laptop pata hard drive that makes a slight constant clicking noise. hard drive is more than likely dead/dying right... i can see that partitions of the hard drive in disk management and i can see the partitions in my computer but i can't get to any of the data.

any suggestions?
