Windows 10: Apc_index_mismatch [bsod]

Hello

Yesterday after coming back to my laptop after about 5 hours I brought it out of standby and whilst doing some work it blue screened with the error APC_INDEX_MISMATCH. I think I was trying to enter the character 1/2 with an ALT code. The error happened 3 times. I ran the Microsoft Security scanner and 4 hours later it found nothing. I ran the WinDbg program but it didn't give an actual cause.

I ran your program and attached it as required. Hope I've provided enough information.

BSOD APC_INDEX_MISMATCH ntoskrnl.exe+142940

I had a BSOD. Bug Check String APC_INDEX_MISMATCH , cause by driver ntoskrnl.exe caused by address ntoskrnl.exe+142940. Adive on the fix for this. XPS 15 9950 RUNNING Windows 10 Pro.

 

OneDrive Causing BSOD: APC_INDEX_MISMATCH (Windows 10 1709)

Hello,

I've figured out the root of this BSOD; when OneDrive is enabled, a BSOD occurs within seconds. The BSOD message displayed is APC_INDEX_MISMATCH. I've tried various methods of repair to no avail.

This is on Windows 10 Ent 1709 (OS Build 06299.125)

Has anyone else experienced this issue?

Hi Bill, agree the dump doesn't show anything useful. The eventlog does though. I see a lot of these:

I've checked my PCI drivers and they are all up to date. I did read somewhere that the Realtek HD Audio driver could be a problem but that's up to date and has never been changed. I did have headphones plugged in at the time and since the last BSOD I've not had another one and the headphones haven't been plugged in.

I'm not sure if this will be any help but the eventlog identifies the vendor and device (8086; 0xA113) checking this in msinfo gives:
Intel(R) 100 Series/C230 Series Chipset Family PCI Express Root Port #4 - A113

This port appears to be shared with the Ethernet network card. That is where I would focus your attention.

I did investigate the shared port but as it's a laptop I couldn't do anything about it. Also the BSOD hasn't happened again.

2) Run memtest86+ version 5.01 for at least 8 passes. Memtest86+ - Advanced Memory Diagnostic Tool
This may take hours so plan to run it overnight.

The more passes the better.
There are a significant number of false negatives if fewer than 8 passes are made.
A false negative is a test pass when there is malfunctioning RAM.

Memtest86+ version 5.01 testing takes approximately 1 - 2 hours /GB RAM
Just 1 error is a fail and you can abort testing.
Then test 1 RAM module at a time in the same DIMM each for 8 or more passes.
b) When Memtest86+ has completed 8 or more passes use a camera or smart phone camera to take a picture and post an image into the thread.

3) Open Ccleaner > click Windows tab > scroll down to system and advanced > post images into the thread

4) There were 6 unexpected restarts after the last dump. What has been happening with hangs, freezes, crashes, etc ?

5) Review these software > uninstall and re-install the software.

Please re-post to include all rows under System
2) For system failure > un-check automatically restart
3) For the file explorer search please search for win32k.sys
Once that folder is available search for any dmp file dated 2018> post into the thread
4) For Memtest86+ version 5.01
Memtest86+ - Advanced Memory Diagnostic Tool
In case there are any problems booting:
1. Try another USB stick
2. Try all USB ports (2.0 and 3.0)
3. Maybe when using UEFI Bios you have to temporarily switch to Legacy in BIOS settings
4. Turn off temporarily fast boot, secure boot, CSM if enabled

- Found C:\cache\win32k.sys but no dmp files there

I've created the bootable USB on 2 different sticks but the BIOS doesn't recognise either in any USB Port. I can't find where in BIOS to temporarily switch to Legacy and fast boot, secure boot, CSM are all disabled.
