Windows 10: Apc_index_mismatch [bsod]

I'm pretty certain I had to have CSM enabled to get my USB to boot.

 

Whilst trying various options I couldn't enable CSM, it was greyed out.

What is seen when you click add new boot option?
What are the options for Windows boot manager?
What are the options for USB configuration?

 

You will also need to temporarily disable UEFI and enable Legacy boot.

 

The first picture show the USB configuration options.
The second picture shows options to add a new boot option.
The third picture, it lists the boot override, if you click the Acronis or Windows Boot manager lines it will boot into those options. If you click the UEFI: SanDisk it does nothing which suggests it doesn't like that option.

 

How did you make your USB key for the Memtest86+ program? Did you follow the instructions in the TenForums tutorial?
Select the download as shown below:




Run the file, select the drive where the USB key is inserted, check the box to format it (formatting it as FAT32) and proceed.

 

Yes followed that tutorial to the letter twice with two different USB sticks.

 

I'm not sure why your system does not see the USB drive and boot from it. Do you have a CD/DVD drive? You can download a bootable ISO instead and burn it to a CD/DVD and try to boot from that.

Is fast startup in Windows disabled?

 

Not disabled.

Fast startup needs to be disabled as well.

 

Yes I tried it with it disabled. I'm going to try a DVD when I get time.
