Windows 10: AppHangB1 explorer.exe crashes several times per day

This problem has occurred mostly since 6/20. 2-4 times per day. I cannot identify anything in particular that I am doing when the crash occurs. Has happened when switching apps with taskbar and after login (after I have stepped away for a while)

I can restart explorer and go on with my day until the next one. Details on each crash are identical.

BAD_POOL_HEADER several times per day

i don't have a make/model because i built it myself. i can attach a speccy file after i install the program again since i have recently re-installed(clean install) windows to see if the problem was due to me upgrading the first time

components are as follows

mb - MSI B85-G41 PC MATE

processor - core i3-4330 3.50GHz

memory - 14GB 4x4GB and one 2gb in dual channel mode (i ran memtest and found no errors)

graphics - AMD radeon hd 5450

HDD - wd blue 250gb sata

windows update states i have a few office related updates, which i am installing now.

issue has not occurred in safe mode, nor my dual-boot ubuntu 15.

have performed refresh several times and reset a few times. there was ONE occasion, long ago when the BSOD threw up a different error, which i feel was graphics card related(i want to say it had amd or ati in the name of the 'offending file' before it blinked
off the screen.

things i have noticed are that sometimes it crashes and the error is readable (gigantic frown -- we are collecting info, etc) but other times, after a certain point, like maybe 1/5 of the way down the screen, the text is scrambled horizontally and unreadable.
you can view the progress, and the pc restarts quickly when the text is readable. but it takes noticeably longer to reboot when the bottom is scrambled.

any help you can give as to why my pc crashes several times per day. usually when watching video/listening to music. this has been going on for over a year, (with win 8.1 AND win 10) the pc went unused from january to march and when i plugged it in, i had
no BSOD's for about a month, but it started doing it again 2 weeks ago. they seem random, but i was hoping maybe someone with greater knowledge could take a look.
