    The primary display goes solid green and the secondary screen goes black. The audio - if playing - repeats the last few milliseconds of whatever was playing just before the screen changed.

    I should mention that this doesn't happen very frequently. Usually once every few days. It has repeated itself twice over the course of a few hours, but never more than that. It has gotten worse recently, but improved slightly when I installed older drivers (the drivers that came with my GPU).

    I'm not sure if my upload will shed any light on the issue... The crash that happened on the 14th wasn't anything different from other crashes (from what I remember), and there have definitely been crashes since then. I check event logs, etc. right after a crash but there are never any errors that look like they're causing the issue. It always seems to be "windows was not shutdown correctly) after I restart and things like that.

    Nevertheless hopefully someone can help me out using what I've uploaded.

    I am finding it hard to continue using EDGE or windows 10. I am unable to view youtube videos at all unless I use private mode.

    Why hasn't someone answered Matthew's question after 2 years? I have searched for 2 weeks and am unable to find a solution anywhere.

    Can you try the 16.3.1 Hotfix CCC display driver and let us know the resulting situation?

    Okay I've installed 16.3.1 and I'll post again if/when the next crash occurs.

    Another crash occurred a few minutes ago, so I ran dm_log_collected.exe again and attached that .zip to the original post (in addition to my earlier .zip).

    edit: a second crash occurred a minute or two after I originally posted this reply

    The zip you uploaded does not contain any new crash dump.

    Follow it: BSOD Minidump - Configure to Create in Windows 10
    Go to Option Two, "Small Memory Dump". Download the .reg file and merge it in registry by double clicking it.

    Now wait for another BSOD. When it occurred, seh the .dmp files manually in the default path: C:\Windows\Minidump or %SystemRoot%\Minidump. See if the crash dump is recorded or not (hopefully it will be recorded).Post it following the Blue Screen of Death (BSOD) Posting Instructions.

    Dont run any disc cleanup tool before you upload another zip.

    I've already configured minidumps and they aren't being recorded. The only one I have is from Jan 14 and there have been several crashes since then.

    edit: 15 minutes after posting this (and after double checking my minidump settings), another crash occurred. The only file in the minidump folder is still the one from Jan. 14

    edit2: 40ish minutes after the first edit another crash occurred. Still no new file in the minidump folder.

    The onscreen codes may give us some info about it.

    I can take a picture the next time it happens, but I don't think it will be much help... it isn't a BSOD screen with information, it's just a solid green #2CE539 screen on the primary monitor and a solid black screen on the secondary.

    Well, then I think it is not a BSOD issue.

    In your event log, I have noticed two most important failures .....

    • User Data Storage_42e17 service
    • AMD ACP Kernel Service

    One thing that made me extremely strange that you are running Prime95 at the startup!!!!
    Code: Prime95 z:\users\kevin\appdata\local\temp\7zo42481751\prime95.exe MOTHERSHIP\Kevin HKU\S-1-5-21-177651987-915648922-3078247699-1006\SOFTWARE\Microsoft\Windows\CurrentVersion\Run[/quote] And it is a most inconvenient location for a startup item, which gives a strong hint of a virus. Or it is some issue relating to moving the user folders in a different location which is acting up to a great extent.
    Free up the startup. Windows does not need any other program to auto-start with it; but they can conflict which may cause various issues including randm crashes including BSODs.

    • Right click on the taskbar and click on "Task Manager" in the context menu.
    • It will open Task Manager with "Processes" tab selected. Click on the "Startup" tab.
    • Select them all one by one and click "Disable" at the bottom right corner.
    • Disable everything there other than the antivirus.
    See how it goes. Let us know.

    Stress test the Graphics Card using Furmark. Upload the screenshot for us. Also let us know if you have experienced any crash/BSOD and/or artifacts during the test.

    Test your RAM modules for possible errors.MemTest86+ - Test RAM Run memtest86+ for at least 8 consecutive passes.

    If it start showing errors/red lines, stop testing. A single error is enough to determine that something is going bad there.
    Take a camera snap of the memtest86+ window before closing the program. Let us see it.

    Stress test the CPU. Prime95 - Stress Test Your CPU It saves the result as a .txt file in the prime95's folder. Upload the file for us.

    Test your HDD with Seatools.SeaTools for DOS and Windows - How to Use - Windows 7 Help Forums

    Let us know the results of all the above. If all goes fine, I will suggest for a full wipe of the HDD and and a clean reinstall of the Operating system.
