Windows 10: Disk usage in task manager always at 100%

I have a HP Pavilion 15 cs1063cl. I have been using it for 4 years. I keep having issues with the disk being 100% all the time. I faced many problems trying to resolve this which brought new problems. But in the end I made a Clean Installation. Even now the disk is still in 100% usage. Is it a hardware issue. I have opened my laptop a few times but I strictly followed the manual. I opened it to add a new RAM card thinking it will help with the issue, to clean off the dust, I even changed the thermal paste, I also changed the battery once since the old one was swollen. I also used to have this
