Windows 10: Sounds keeps gradually muting

So i've had this issue only when using the speakers in the laptop, whenever i use headphones it doesnt happen. Whenever watching youtube, netflix, itunes, etc, the volume will say it's on max but will automatically and slowly mute itself. Then after a minute of being nearly muted, it goes back to full volume and repeats. I have no idea how to fix this and it's driving me crazy, any ideas would be awesome. I've tried rolling back the driver and that didnt help

Why does Windows 10 unmute my system sounds?

Ever since I installed Windows 10 I keep hearing system sounds. I go to the volume mixer to mute the sounds and it works for a while but when I log out, turn my computer off, or update it automatically un-mutes the system sounds. Is there anyway to fix
this and make the mute permanent?

 

Windows 10 Volume Lowers Itself

This has happened three times in the last few hours. I've been listening to music on itunes, when the volume will gradually mute itself. It's as though I'm holding down the lower volume button on my keyboard but I'm not. I have a usb keyboard, so I move
it out of the way to see my actual laptop keyboard, and there doesn't seem to be anything wrong with it. My keyboard wasn't pressing on the button or anything.

I can't raise the volume by just clicking the sound icon on the taskbar, I have to click the mute/unmute button on my laptops keyboard to turn the volume back on. If I don't when I try to use the sound icon, it will just scroll back to 0 sound as soon as
I stop moving the slider.
