    Hello,

    I've recently begun experiencing BSOD's when I start up my computer for the first time every day. My machine goes into a boot loop every time and several BSOD errors show up. I keep a log in an excel file and I've attached a pic;

    This is up to date and has happened today. I can only seem to boot far enough to use the computer if I do a restore or a reset. I don't want to do a reset every time so I choose restore, and now I've resorted to creating a restore point every night before shutting down, but manual restore points seem to have a 50/50 shot of succeeding. Any and all help will be greatly appreciated! I've attached the zip per the instructions here.

    WHEA UNCORRECTABLE ERROR BSOD in Windows 10

    I have been experiencing multiple BSOD screens throughout the day, yesterday.

    It happened while performing a graphic rendering when system got frozen, and after a hard resetting I got the first BSOD.

    After that, computer got into a resetting loop, and when finally load into Windows, it could not be shut down completely.

    I tried to solve the problem by restoring the entire disk image, rolling everything back to 11/3/15, but without results. BSOD kept showing.

    Please, I would appreciate your assistance on this, so I am enclosing the link to the Minidump files together with the list of my PC components.

  3. Windows 10 BSOD on new PC

    I just recently built a PC and installed Windows 10 Home 64-bit onto it. I've had multiple problems with it, including several BSODs. I've tried reinstalling Windows 2 or 3 times, resetting the PC, and just recently used the installation media to restore
    my PC using the media creation tool and a flash drive. I've tried keeping track of all of the error messages I've gotten on BSODs and they are

    - Reference by pointer

    - Bad pool header

    - Kernel security check failure

    - System thread exception not handled

    - Security check failure

    - System service exception

    Any help would be greatly appreciated.

    As these crashes are happening during the boot process there are no crash dump files being created so we can't provide any debugging for you. It is probably due to bad driver (3rd party, not Microsoft) and so I would recommend that you try a clean boot process and if a clean boot is troublefree you can then investigate which of your non-Microsoft startup services or apps might be causing the problem.

    Also,

    Try to boot your computer with the least amount of peripherals possible. So disconnect all USB devices other than mouse and keyboard.
    So printers, USB-sticks, ...

  6. Booted with only the mouse and keyboard attached. Got a KERNEL SECURITY CHECK FAILURE and a SYSTEM SERVICE EXCEPTION message and the 3rd reboot got back into windows.



    After I got back into windows from the above boot troublesehoot, completed the clean boot process as per the tutorial, then rebooted and got two consecutive IO1 INITIALIZATION FAILED messages before it booted into automatic repair and from then I restored to a point I had from yesterday.

    1) Are you files backed up?
    2) Had you made an image of the drive?
    3) How old is the computer?
    4) Are all of the components the same age or have some components been replaced?
    5) Had you ran any tests on the drive: SMART, short or long generic. health, benchmark, or error scan?
    6) If you needed to would you be able to perform a clean install (delete partitions, format drive, make unallocated drive, install windows 10 version 1703 using a windows 10 iso)?
    7) Can you open file explorer?
    8) Can you reliably open windows advanced troubleshooting menu?
    9) What was the result for each of these:
    a) startup repair
    b) reset with save files
    c) reset with remove files
    d) startup options with safe mode with command prompt
    e) command prompt
    10) Had you tried a registry repair?
    11) Had you tried using a bootable windows 10 iso?


    12) Update the system specs in "My Computer"

    13) Make sure that there is no over clocking while troubleshooting.

    Sometimes there are problems in the bios that produce bsod.

    15) To ensure that there are no improper bios settings please reset the bios.

    Sometimes there can be failure to boot after resetting the bios.

    16) So please make sure your files are backed up.

    This sounds like you only got so far with the clean boot process. Did it boot successfully with all 3rd party drivers disabled? If so then that is significant. When you rebooted had you added back any startup services as described in the part 2 of the clean boot process? Completing part 2 and part 3 of the clean boot process is what identifies the services or drivers causing the boot failure.

    If it failed to boot with the clean boot process this would suggest you might have a hardware problem. You now need to see if you can boot into safe mode. We need to know if it will run in safe mode for a period of say 1 hour to check if the hardware is OK.

    The initialization error is indicating that something got corrupted causing the initialization of the I/O to fail. Hopefully the repair has fixed that.

  10. Hello, sorry it took so long to respond, been busy. I created a backup system image and did a clean install thinking it would work, and today I get more errors (question 6). Does this increase the possibility of a hardware issue? Hopefully it's a failing SSD because I have a spare.

    I've answered zbook's post below.


    1)
    Are your files backed up?
    a. Yes.

    2) Had you made an image of the drive?
    a. Yes.

    3) How old is the computer?
    a. Custom built, most components about 6-7 years old. I still use an Ivy Bridge i5-3570K.

    4) Are all of the components the same age or have some components been replaced?
    a. The power supply has been replaced as I though the previous one died, but didn’t, there’s a whole story there. The graphics card has been updated twice I believe. Currently an EVGA 970.

    5) Had you ran any tests on the drive: SMART, short or long generic. health, benchmark, or error scan?
    a. Yes, I launched into UBCD & ran parted magic. I know SSD’s don’t read very well with SMART, but I did complete the available short test an all drives, and all passed.

    6) If you needed to would you be able to perform a clean install (delete partitions, format drive, make unallocated drive, install windows 10 version 1703 using a Windows 10 ISO)?
    a. Actually I just did this last night, 10/2, and I today I still got the following errors;
    PFN_LIST_CORRUPT PFN_LIST_CORRUPT BAD_SYSTEM_CONFIG_INFO 0xc000021a (several times) If this isn’t fixed I’m sure I’ll get more tomorrow.

    7) Can you open file explorer?
    a. Yes, it opens.

    8) Can you reliably open windows advanced troubleshooting menu?
    a. Yes, unfortunately it opens each time I start up the machine for the day after passing 5 failed boots, each one producing a bluescreen.





    9) What was the result for each of these:
    a) startup repair
    Couldn’t repair your PC.
    b) reset with save files
    I’ve tried this before posting here & it didn’t work.
    c) reset with remove files
    AKA clean install, see question 6.
    d) startup options with safe mode with command prompt
    e) command prompt
    It opens, I’m not sure the result we want here. I’ve done sfc /scannow, and chkdsk. There is one drive that shows 7 errors, and that’s the one that can’t be locked, which I assume is the boot drive.
    9) Had you tried a registry repair?
    a. Not sure what that is, most say there’s no such thing and registry cleaners do more harm than good.

    10) Had you tried using a bootable Windows 10 ISO?
    a. And doing what? I’ve already done a clean install as per question 6.

    11) Update the system specs in "My Computer"
    a. Updated systems specs.

    12) Make sure that there is no over clocking while troubleshooting.
    a. I’ve never overclocked.

    Sometimes there are problems in the bios that produce bsod.

    13) Please check to see if this is the most up to date version.
    a. Yes, according to the ASRock website, 2.90 is the latest version.


    14) To ensure that there are no improper bios settings please reset the bios.
    a. I’ll get around to that and respond later.

    15) Find a flash drive that you can format & create a bootable Windows 10 ISO:
    a. I’ve already made one of these while doing the clean install, see question 6.

    The event log displayed problems with drive corruption.
    These are the same problems in the event viewer before the clean install.

    If you can boot to the desktop please open administrative command prompt and type or copy and paste:
    1) sfc /scannow
    2) dism /online /cleanup-image /restorehealth
    3) When these have completed > right click on the top bar or title bar of the administrative command prompt box > left click on edit then select all > right click on the top bar again > left click on edit then copy > paste into the thread
    4) chkdsk /x /f /r
    (This may take many hours to run so plan to run overnight)

    Using safe mode if necessary run HD Tune:
    HD Tune website
    to check the health,
    scan for errors, no quick scan but full scan
    run a benchmark.


    It may take some time, but please take the time you need to perform it properly.
    When above is done please make screenshots of the following

    the error scan,
    the benchmark incl. following
    transfer rate,
    access time,
    burst rate,
    cpu usage.

    The only drivers I installed myself was video drivers, nvidia's geforce experience and then a driver update.

    Hey so I've gotten a few more blue screens recently, got a picture below;

    For a moment I thought I had gotten a bad windows update, as I've heard of those and maybe one finally found its way to me, as I had gotten an update before it got worse, and just a couple of days ago I got another update which resulted in less blue screens, but that changed when it all started up again.

    Another idea I had was a conflicting program, since I started getting errors a day or so after that clean install, so I uninstalled the last program I had installed the night before, which was Plex media server, but just today I get more blue screens so evidently that isn't the case.

    What I do now is create a restore point before shutting off for the night, and if it's unable to eventually boot to desktop I would just restore to the point I had made the night before. Some more interesting things that I hadn't mentioned previously is that during the first boot attempt of the day it shuts off immediately after POST, then I have to turn it on again for it to progress further and either blue screen or get to desktop. Another irregularity I've noted is before the clean install and before the resets, when it was really bad I wouldn't be able to open the start menu, the notifications tab on the right, or the pc-tools-thing when you right-click the start button. I would have to turn the computer off via the ctrl-alt-delete menu.

    So anyway I have the latest error log attached. I'm still here, still experiencing this issue, any help is appreciated. I'm about to do some hardware troubleshooting as that seems more likely than software at this point, going to try booting with a different PSU, then work down the list I have which is different video card, different RAM, and finally different SSD.

    If all fail, I think I may declare it dead-or-dying and just upgrade. It's been six years since I've put together this machine and I've been eyeing those new coffee lake CPU's...

    There were no bsod mini dump logs.
    The event logs indicated that they failed.
    Were you able to run chkdsk /x /f /r?
    Please post the event.vwr result into the thread:

    In the left lower corner search type: system > on the left pane click advanced system settings > in startup and recovery click settings > post an image into the thread.

    If automatically restart is checked > un-check automatically restart
    Change write debugging information to automatic memory dump and dump file to %SystemRoot%\MEMORY.DMP

    Open administrative command prompt and paste all of these command together:

    net stop wuauserv
    cd %systemroot%\SoftwareDistribution
    ren Download Download.old
    net start wuauserv
    net stop bits
    net start bits
    net stop cryptsvc
    cd %systemroot%\system32
    ren catroot2 catroot2old
    net start cryptsvc

    Then type or copy and paste:
    1) dism /online /cleanup-image /checkhealth
    2) dism /online /cleanup-image /scanhealth
    3) dism /online /cleanup-image /restorehealth
    4)When these have completed > right click on the top bar or title bar of the administrative command prompt box > left click on edit then select all > right click on the top bar again > left click on edit then copy > paste into the thread
